All mail servers should have a reverse DNS (PTR) entry for each IP address (RFC 1912). Missing reverse DNS entries will make many mail servers to reject your e-mails or mark them as SPAM.

All IP's reverse DNS entries should resolve back to IP address (IP → PTR → IP). Many mail servers are configured to reject e-mails from IPs with inconsistent reverse DNS configuration.

You must setup your rDNS with your provider

Some providers have this settings in there control panel, others ask you send them a ticket

make sure you have setup a A record before setting up your rDNS

Setting up a A record is easy , just add your servers hostname (example serv1.hosting.com) as A record
Example: serv1 A 111.1.1.1<----Here your ip address

and this one also shows the .in-addr.arpa.: http://www.dnswatch.info/ (just enter IP)
SPF/PTR creator: http://www.spfwizard.net/
Log into panel as zadmin, go to DNS Manager > Select domain you use for your mail server and add the PTR record created from above as an SPF record: (example)

Target = v=spf1 a mx ptr ip4:[xxx.xxx.xxx.xxx] mx:mail.[domain].com include:smtp.[relayer].net -all
Of course replace everything inside the [] brackets (also remove the brackets!) with your information.

I have even fixed reverse dns issue.

Ip Reputaion- Is good and not listed is any  blacklist

Still all the mail sent are marked as spam.

Sometimes Gmail and Hotmail mark some IP ranges for unknown reasons (most related to previous owners spam reputation), when this happens there is little you can do, however I know of reports that sending email to multiple gmail address and marking the emails as "Not Spam" does help bringing your reputation back up (do this carefully or you might end up in a worse situation).
Like I've told you on my first reply, Gmail (and hotmail, yahoo etc) use a different spam blacklist (you can't check it) and they also blacklist some IP ranges or ISPs.

From my experience (because I had a similar case recently), it has to do with your IP reputation, probably you are using some provider that is known for spam.

All other Blacklists are Clean (http://whatismyipaddress.com/blacklist-check) however it might mean that this ip (or ip range) sent spam before it was on your "hands" and that Google marked it and never deleted it.

Easier will be to buy another IP...
This is probably the issue, I'm almost sure of it.
Try with another IP if you can.

I do not think this might be the issue, however it can be, seems it is not yet fixed.

I made a clean installation in a VPS.
Fwd/Rev DNS Match - Yes,spamcheck.postmark-clean,mail-tester score-9/10,

Domain-ezio.co.in

I have tried with new ip even I have even changed service provider.

Every  Score is clean but still the problem exist. What am I missing?
