I have a begin of idea (but not yet the full cause or solution):
The "ErrorDocument" vhost entries are created from the content of directory _errorpages in the domain.
Can you get a look about what is inside your directories for errorpages in
/etc/sentora/hostdata/[sentora-user]/public_html/[domain]/_errorpages/ ?
I bet there are here duplicate file for each error page.
I do not know why, but could it be possible you have created the domain, then deleted it and then created it again ?
In such case, the errorpage may have been created 2 times. I am not sure about it, but could it be possible that new files created duplicate files suffixed with (1), (2) etc rather than owerwite oldest ?
