Hacked - Uploading File Automatically

Hi,

I am facing this problem a lot. My VPS provider is warning me again and again about termination.

Some php files and folders are automatically created / uploaded in my server they contents fishing scripts. I have strong root and ftp password. But don't know who is uploading and how uploading. Need solution.
